Agreement to Terms

By enrolling in Hamptons Concierge Medicine, you agree to these Terms and Conditions. This agreement governs the relationship between you (the "Member") and Hamptons Concierge Medicine (the "Practice").

Membership Services

As a member, you will receive:

  • 24/7 direct access to your physician
  • Same-day or next-day appointments for acute concerns
  • Comprehensive annual physical examination and preventative care plan
  • Personalized care coordination with specialists and facilities
  • Unhurried, extended appointment times
  • Hospital care management if admitted to Stony Brook Southampton Hospital

House calls and certain other premium services are available for an additional fee.

Membership Fees

Membership requires payment of an annual retainer fee. This fee covers concierge services and enhanced access but does not cover:

  • Third-party medical services (laboratory tests, imaging, specialist consultations)
  • Hospital charges or facility fees
  • Prescription medications
  • House calls or after-hours residential visits

Members are responsible for maintaining health insurance to cover these additional medical services.

Limited Enrollment

To maintain the highest level of personalized care and accessibility, the Practice maintains a limited patient panel. Membership is subject to availability.

Patient Eligibility

Hamptons Concierge Medicine provides care exclusively to adult patients aged 20 years and above.

Termination

Either party may terminate this agreement with 30 days' written notice. Membership fees are non-refundable except in cases where the Practice is unable to fulfill its obligations.

Insurance and Billing

The Practice does not bill insurance for the annual membership fee. However, the Practice will work with your insurance provider for covered medical services when applicable. Members remain responsible for understanding their insurance coverage and any out-of-pocket costs.

Emergency Care

In the event of a medical emergency, Members should call 911 or proceed to the nearest emergency department. While your physician is available 24/7 for consultation, concierge services do not replace emergency medical services.

Limitation of Liability

The Practice provides medical services with the highest standard of care. However, we cannot guarantee specific health outcomes. Members agree that the Practice's liability is limited to the extent permitted by law.

Changes to Terms

The Practice reserves the right to modify these Terms and Conditions with reasonable notice to Members. Continued membership after such modifications constitutes acceptance of the updated terms.
